Record Retention, Release Authorizations & Correspondence

The Office of Access & Equity: Disability Resources (ODR) adheres to the UNC System General Schedule for the retention of records (GU323); therefore, records are maintained for 5 years after the last semester of enrollment. ODR records are not a part of the academic record maintained within the Registrar's Office.

The release of ODR records is governed by the Family Educational Rights and Privacy Act (FERPA). An explanation of FERPA and policies governing release of records may be accessed at the Office of the Registrar.

Permission to discuss disability-related information would be established via the University's FERPA Access option available through AppalNET.

 

Procedure

In order to request information be released, submit a Docusign Authorization Request. ODR will then send a Docusign Release of Information Authorization to the  email account provided for completion. Once the Release Authorization Form is received and processed, materials will be released via the method requested. Please allow up to two weeks for materials to be gathered and released.

Release methods include:

  • Pick up within ODR (photo ID required)

  • Filelocker to an email address verifiable within Banner

  • Mail

  • Fax
